Personalise the notebook

Line 1: required, up to 12 characters.

Lines 2 to 6: optional, up to 12 characters per line.

Lines 7 and 8: optional, up to 20 characters per line.
All your wording will be printed in UPPERCASE (capital letters), regardless of how you type it. The two final lines allow more characters than the first six. Leave unused lines blank and check all spelling before ordering.
