Line 1: required, up to 40 characters.

Line 2: optional, up to 25 characters; this line appears in larger lettering.

Lines 3, 4 and 5: optional, up to 40 characters each.
Your wording keeps the capital and lowercase letters you type. Avoid writing line 2 entirely in capitals because the larger lettering can become distorted; use Emily rather than EMILY. Leave unused lines blank and check names, dates and spelling carefully.
